Terms of eligibility for practising privileges at Solasta Healthcare

The application process for Medical Staff to apply for Practicing Privileges at Solasta Healthcare, includes the requirement to:

  1. Complete and sign the Practising Privileges Application Form for Solasta Healthcare
  2. Provide a copy of
  • Completed application form
  • GMC Registration Certificate
  • Full copy of Annual Appraisal
  • Indemnity Insurance Certificate (to include private practice)
  • Enhanced Disclosure certificate through AccessNI – we assist with this process
  • References x 2
  • Curriculum Vitae
  • Copy of Hepatitis B Status
  • Registration with the Information Commissioner’s Office (ICO)
  • Evidence of required Mandatory Training

3. You will be required to visit clinic to meet with Clinic / Medical Director

GMC, Medical Indemnity Certificates and Appraisal Forms are presented to the clinic on a yearly basis, as they are renewed.  If this information is not available, sanctions will be applied which will prevent doctors from practising at Solasta Healthcare.
