
Terms of eligibility for practising privileges at Solasta Healthcare
The application process for Medical Staff to apply for Practicing Privileges at Solasta Healthcare, includes the requirement to:
- Complete and sign the Practising Privileges Application Form for Solasta Healthcare
- Provide a copy of
- Completed application form
- GMC Registration Certificate
- Full copy of Annual Appraisal
- Indemnity Insurance Certificate (to include private practice)
- Enhanced Disclosure certificate through AccessNI – we assist with this process
- References x 2
- Curriculum Vitae
- Copy of Hepatitis B Status
- Registration with the Information Commissioner’s Office (ICO)
- Evidence of required Mandatory Training
3. You will be required to visit clinic to meet with Clinic / Medical Director
GMC, Medical Indemnity Certificates and Appraisal Forms are presented to the clinic on a yearly basis, as they are renewed. If this information is not available, sanctions will be applied which will prevent doctors from practising at Solasta Healthcare.
